Our New Online Community is Going Strong!

Healthier Living Alumni: A Community for Graduates of the Chronic Disease Self-Management Program (CDSMP)

We are happy to announce the launch of our new on-line community: Healthier Living Alumni. We have recruited 100 members as of today, a breakthrough after many months of hard work by the BTOP/OOA team and NCOA.

San Francisco’s Department of Aging and Adult Services (DAAS) in partnership with National Council on Aging ( NCOA), worked to provide an on-line support community for graduates of the Chronic Disease Self-Management Program (CDSMP) from local communities or the Better Choices, Better Health® online workshops.
